To request a travel permit, please forward the following information via email to the link provided at the end of these instructions:

Once the information is received and team registration is verified, you will be sent a signed travel permit to the fax number listed or it will be sent by regular mail.  If you wish to make other arrangements to receive your travel permit, please state this on your request.  If you are requesting travel permits for more than one tournament, simply list all the tournaments and we will issue one travel permit for each.
